Connect with us wherever you go. You can safely and securely access MyChart from youriPhone or Android.

MyChart is our free online portal offering patients personalized and secure access to portions of your medical records, from the comfort of your own home. Join the new generation of patients taking a more active role in managing their health and wellness.

USF Health strongly encourages all patients to sign up for MyChart, all you need to do is request an activation code.
This code will enable you to log in andcreate your own user ID and password.

The activation code can be created by:

  • Request a code from your healthcare team at your next appointment, in person or via Telehealth

The 21st Century Cares Act: What this means for you

USF Health patients will now haveexpanded MyChart-based access to most of their clinical data. A new federal rule called the21st Century Cures Act, which is going into effect in 2021 is designed togive patients more control over their personal health information.

Starting on Nov. 2, 2020, USF Health will provide nearly allphysician-generated clinical documentation related to your visits with USFHealth providers and release them to you via MyChart. Once your physiciancompletes the notes and documentation associated with your USF Healthappointment, those details will automatically be sent to you’re your MyChartaccount.

This more complete documentation will include lab results, imagingresults, and pathology results, along with completed notes and orders relatedto your visits with USF Health providers.

Communicate with your medical care team

Communicate with your provider and ask non-urgentmedical questions from the comfort of your own home. Forurgent medical matters, please contact your doctor’s officeor dial 911 if it is an emergency.

Request and Manage Appointments

Schedule your next appointment, or view detailsof your past and upcoming appointments.

View your health summary, including routine test results

Share labs: Send your lab results to your primary care provider and others on your health team.

Print Lab Orders

You may now print certain lab order requisitions from MyChart where a download button is available. Click herefor detailed instructions.

You can view the latest statements and make online payments

Manage your billing account wherever yougo, at any time. You can view the latest statements andmake online payments 24/7. You may also submit billingquestions online to our patient services team.

Request prescription renewals
MyChart will also give you access to the most trusted health information resources.

Now offering eCheck-in feature onMyChart, our online patient portal.

Now available through MyChart, eCheck-in allows you totoconveniently:

  • Pay visit copays
  • Update your demographics
  • Verify your medical insurance information
  • Respond to questionnaires
  • Electronically sign consents
  • Limit contact when possible


How do I useeCheck-In?

When youreceive your reminder notice about an upcoming appointment, you will be askedtolog in to MyChartand select the appropriate upcomingappointment. Click on the UPDATE INFORMATION button to begin eCheck-Inprocess.

Not onMyChart? Go to MyChart and click the “sign up” button. Once you complete the process, please stopby the front desk to let us know you’ve arrived. eCheck-in online at home or bysmartphone and save time when you arrive at our clinic for your appointment.

If you are connecting viaTelehealth, also known as a video visit, please complete eCheck-in before yourappointment.

USF Health + Tampa General Hospital

Our affiliation with Tampa General Hospital givesour patients access to their health records from bothorganizations through one MyChart Account.If you currently have a MyChart account with Tampa General hospital, you do not need an account at USF Health.

Your privacy is important to us

We take great care to ensure your health informationis kept private and secure. Access to information iscontrolled through secure activation codes, personaluser names, and passwords. Each person controls theirpassword, and the account cannot be accessed withoutthat password. Unlike conventional email, all MyChartmessaging is done while you are securely logged on toour website.
